While your health and fitness is our number one priority, Sylvania Family Physicians also offers a number of services centered on your health care. Should you have any questions about the services listed below, do not hesitate to call us at 419-885-0900.

Prescription Refills
We offline online a prescription refill service.
You also may contact our office to refill your prescription by calling our 419-885-0900. When calling, please wait for the prompt and choose the appropriate option for a medication refill. You should include your name, medication, dosage, prescription number (if known), quantity and your pharmacy’s telephone number.

Please allow 24 hours notice for medication refills

Billing Services
Our billing is coordinated by Provider Solutions of Northwest Ohio. If you have any questions about your monthly statement or an insurance claim, please call 419-861-7600 to speak to a representative.

Referrals
If your insurance company requires that you have a referral prior to seeing a specialist or receiving certain medical services, please notify our office as soon as possible at 419-885-0900.
When calling, wait for the prompt and choose the appropriate option for a referral. You should include your name, the name of the specialist, the appointment date and your insurance information.

Messages left for referrals are retrieved frequently throughout our open office hours.

Disability/Off Work Forms
Typically, our office requires 5-7 business days to complete forms. If you cannot wait the 5-7 business days, you will be charged a $20 fee for completed disability/off work forms.

Test Results
Sylvania Family Physicians handles communication of your test results two ways. You will either receive a letter in the mail regarding your results, or a member of our clinical staff will call you.
If you do not receive some form of communication from us within a week after your test(s), please call us at 419-885-0900.

Electronic Medical Records
Sylvania Family Physicians has been using electronic medical records since early 2004. Using EMR has many benefits for you. With EMR, there are no more paper charts to have to keep track of and we can access patients’ files at any time.

In addition, we can provide patients with information on-demand about their specific diagnosis or any medications prescribed.
