I've never met either Dr. Agee or Dr. Laungani. How do I know if I'll like either physician?

How do I become a patient of the Sylvania Family Physicians?
Call the office to set up an appointment. Before you can become a new patient, we can do our best to help you only if you first complete new patient paperwork, including:
- Patient information, including medical history
- Consent to communicate

- Consent to treat minors (when applicable)
The above forms should be completed prior to your appointment. We ask new patients to arrive 20 minutes before their appointment time.
Do I need to notify your office of an address change?
Yes, it is important for you to notify our office and your health plan of any address change. This will assure that your authorizations will be sent to you in a timely manner and avoid possible scheduling delays. You may contact us with your address change by calling us at 419-885-0900, or by faxing your request to us at 419-824-6447. Please include your name, new address and your phone number on all correspondence.
What do I need to know about making an appointment with Sylvania Family Physicians?
To schedule an appointment, call us at 419-885-0900 during regular office hours or Schedule an Appointment via our Website. Please have available your health plan card to ensure optimum service. A reminder: if this is your first visit, please bring a list of your medications and have knowledge of your medical history.
If you are sick, don’t hesitate to call us. We save time every day to see patients for same-day sick calls. If you need a same-day appointment, please try to call as early in the day as possible.
What happens if I miss my appointment or cancel at the last minute?
Sylvania Family Physicians require 24 hours notice to cancel or reschedule an appointment. If you do not appear for the scheduled appointment, we will mail you a letter to remind you of your missed appointment and the need to reschedule. There will be a $20 No Show Fee charged to your account.
If you have multiple no call/no show appointments, then we will send you a detailed letter that outlines our office policy and that, if you continue to miss appointments or cancel within 24 hours, we may discharge you from our practice.
What health insurance plans do you accept?
We accept the major health care plans available in the Toledo metro region; more than a dozen currently. If you are interested in becoming a patient, please first contact your insurance company directly or refer to your insurance handbook to verify that Dr. Agee and/or Dr. Laungani are providers with your insurance.
What hospitals do you use?
Dr. Agee and Dr. Laungani currently do not attend to patients at any of the area’s hospitals. When one of our patients is in the hospital, we are in contact with the attending physician to provide any needed information on the patient’s health history, and to coordinate any care required after the patient is discharged.
Do you handle Bureau of Workers’ Compensation claims?
No. Sylvania Family Physicians doctors are not providers for BWC claims.
Why do I have to see my primary care physician (PCP) before seeing a specialist?
Your primary care physician – either Dr. Agee or Dr. Laungani – knows your medical history. Either one can evaluate your medical condition and determine when your diagnosis or your medical problem requires the care of another specialist.
What do I do in the event of an emergency?
If it is a life threatening or other serious medical situation that you feel requires immediate medical attention, call 911 immediately or go to the nearest emergency department.
